Adolescent Transitions Project Kicks Off with Adult Primary Care Survey

Providers attending AHP’s clinical grand rounds in March learned about the Adolescent Transitions project for managing the transition of young adults from pediatric care to an adult model of care. Among the many benefits associated with improving this process is improvement in the continuity of care and management of chronic diseases for this population.

A survey to gauge the willingness of adult providers across the network to accept new, young adult patients is the first step for the Transitions project. Additionally, the survey seeks to identify providers willing to accept members of this population with complex medical issues, as well identify the perceived barriers to accepting these patients for providers who cannot.

AHP family medicine, med peds, and internal medicine providers can expect to receive this survey via email the week of May 2nd.
