Reminder: Return Your Fidelis Contract Opt-In Form
January 29th, 2018 | Network NewsProviders were notified in December of AHP’s fee-for-service contract with Fidelis Care of New York This new contract offers enhanced rates to AHP providers, both primary care and specialists.
To take advantage of these enhanced rates, providers who already participate with Fidelis need only to return the opt-in form that was mailed earlier this month. If your practice does not currently participate with Fidelis but wishes to, there will be a 60 – 90 day credentialing process. Not interested? No need to take any action: you will not be enrolled if we do not receive your opt-in documentation.
