HCC Pilot Gearing Up for Year Two

Improving processes around hierarchical condition coding (HCC) for Medicare beneficiaries in order to ensure that the acuity of these patients’ conditions is captured year over year is an important initiative for AHP in 2017. The HCC pilot launched in 2016, and representatives from Excellus’ Revenue Integrity team have spent the past few months meeting with participants in the HCC pilot to present findings from 2016 quality reviews and overall scorecard performance.

Pilot participants can expect new scorecards to be released during the first week in May, and quality reviews will be ongoing throughout the year. New for 2017 is the option for practices to either receive their pre-printed scorecards from Excellus, or they may access their initial and updated scorecards via an online portal. In addition to the portal option, enhancements have been made to the scorecards and the supplemental files which are expected to streamline HCC workflows and reduce some of the administrative burden for staff. Following the 2017 pilot kickoff in May, AHP and Excellus will evaluate next steps for expanding the HCC pilot to other primary care practices in the employed and community practices.
