AHP has afforded me the opportunity to maximize care management. I have been able to complete PCMH requirements with excellent support. The ability to improve our metric requirements through tracking and recording is a very needed asset. AHP is improving child health in our community by fostering collaboration between primary care clinicians and specialists who care for children and adolescents. They have initiated an exciting, effective, and evolving model of Care Management that pairs RNs and LCSWs with private practices. Our practice can attest to the significant improvement these Care Managers bring to our practice medical home and our ability to improve care delivery to our patients and their families. AHP has also been willing to invest in upstream interventions targeting both physical and emotional health of children in our community. We are impressed and grateful for the priority they have given to making a difference for all children and youth in the Rochester region and look forward to the good work we will do together in future years.
