Population Health Report Shows Positive Trends
June 28th, 2018 | Network NewsIn this month’s population health update AHP continues to maintain improvement in the preventive and adult disease management categories and top percentile performance in the pediatric and adult acute measures. With newly focused efforts on closing gaps for pediatric well care visits and lead screening, we hope to see the improvement trend for pediatrics preventive translate to the next threshold in percentile performance in these measures.
