2017 NCQA PCMH Standards Available
July 26th, 2017 | Network NewsHelping practices obtain Patient-Centered Medical Home (PCMH) recognition has been an important focus of AHP since its inception. With over 121 primary care clinic locations recognized or in process to receive recognition, 75% of AHP’s primary care is delivered in the PCMH model that emphasizes patient-centeredness and care coordination.
The final wave of practices seeking recognition under 2014 standards is underway, and AHP will be evaluating its approach for assisting practices with recognition under the new 2017 standards. Practices can expect to hear more in the early fall, but printed copies of the 2017 standards are available from AHP today. Contact Renée Sutton to obtain a copy.
