2019 Optimization Team Focused Measures

As the New Year begins the AHP Optimization Team has started to shift gears in its focus on measures and projects. The team will continue to focus on colorectal cancer screening and fall risk, and add breast cancer screening, well child 3-6 years, and hypertension blood pressure control to its list of measures to optimize. The team will also be initializing a diabetic eye exam “reboot” in 2019 in an effort to reengage the network on that measure.
