Continuing to Expand: AHP’s Behavioral Health Integration Team
April 2nd, 2018 | ArchiveOver the past year our Behavioral Health Integration Team, led by Dr. Tziporah Rosenberg, has delivered educational programming twice a month to our community primary care Care Managers. The training Dr. Rosenberg delivers is centered on behavioral health activation, focused cognitive health interventions and problem-solving therapy to employ when they are working with patients around chronic disease management. These sessions, in addition to the annual Care Management Summit that was just held last month, and the monthly care manager-focused learning collaboratives during which groups review cases and receive feedback from the Behavioral Health team, have all shown terrific results.
As a network, if we improve behavioral health care for the population we will see improvement in overall health and likely downstream cost reduction. In order to better track and quantify the utilization and downstream effects of these and other care management efforts, AHP will be implementing our Arcadia-based Care Management Platform this summer which will allow for more systematic tracking and ability to assess impact.
