AHP’s commitment to improving health equity guides our partnership with our rural hospitals and practices to build and maintain a program that supports families with breastfeeding and optimal infant nutrition. Access to International Board Certified Lactation Consultants (IBCLC) significantly improves infant feeding outcomes. Rural communities in our region face substantial barriers to accessing such care.

AHP’s lactation care management program is guided by a transitions care management model of care delivery and supports IBCLC’s commitment to improving health equity to serve rural communities and provide patients with access to seamless lactation care initiated in their obstetric care setting, and then during hospital delivery, as well as throughout their transition to ambulatory pediatric care.

The program substantially improves human milk feeding initiation and duration rates compared to baseline regional and national benchmarks. Patients report improved lactation experiences in obstetric, in-patient, and pediatric settings, while health care providers in these settings report more satisfying experiences of care delivery.

Our team is proud to announce that our data is published by the Journal of Human Lactation, validating the significance of this model of care delivery in improving patient and population health.

Congratulations to Katie Wightman, Lactation Care Management Program Director, and Paige Lounsberry, Lactation Care Manager, on their dedication to this program and the significant effort contributed toward its publication
