AHP Health Catalyst Update
March 3rd, 2025 | NewsAccountable Health Partners is actively progressing with our phased transition to Health Catalyst, our new population health management partner. The AHP team is collaborating closely with Health Catalyst to explore its features, improve data sharing across our network, and enhance our interactions with practices and providers to achieve our shared goal of improving patient care quality within our community.
Most practices were connected to Health Catalyst in 2024, and the remaining practices are on track to be integrated in the coming months. AHP will continue to reach out to practices throughout this transition. We kindly ask that you promptly respond to any requests related to your practice’s transition to Health Catalyst.
