AHP Partners with Pearl Health to Implement the ACO REACH Model

AHP is excited to announce a new partnership with Pearl Health that will enable participation in the Centers for Medicare and Medicaid Services (CMS) ACO REACH model for the 2026 performance year. Launched by CMS in 2022, ACO REACH (Accountable Care Organization Realizing Equity, Access, and Community Health) is designed to improve outcomes for patients in traditional Medicare. This value-based care initiative for Medicare fee-for-service beneficiaries emphasizes health equity, aims to close care gaps for underserved populations, and promotes high-quality, coordinated care delivery.

The decision to enter into the ACO REACH model aligns with our shared network goals of improving quality and costs for patients.

Who is Participating?

This summer, AHP and Pearl Health partnered closely to identify practices and providers in our network who were well-positioned to have success in the ACO REACH model. We worked closely throughout the month of August to identify these practices and prepare for CMS’ September 8, 2025 submission deadline. In total, eighteen AHP groups in private and employed practices will participate in the opportunity.

AHP is working with Pearl Health to design a set of care management activities to best meet the needs and manage the care of patients who qualify for this program. As next steps are identified, AHP will reach out to members of the clinical and administrative teams for participating groups to move forward.
