AHP Team Member Assignments
August 23rd, 2019 | Network NewsAs our network has grown and our activities expanded, our AHP leadership team has decided it will be most effective to create liaisons for each practice site to help with engagement, issues that may arise, and the strengthen lines of communication. To this end, we have developed a spreadsheet identifying the AHP team member who will serve as the primary contact for each practice site in the areas of Medical Director, Pharmacy, Care Management and Provider Relations.
The entire AHP team remains at the ready to assist our members in any way possible, however we hope that this approach will provide continuity and help build collaborative relationships over time as our network and efforts expand. Please see the attached list for your practice liaisons and their contact information within each domain.
