Primary care practices that participate in all AHP gain sharing contracts are eligible to apply for incentive funding for infrastructure and initiatives that contribute to quality performance improvement.
The continuous quality improvement application should be used to apply for an award for a completed quality improvement project, the goal of which was to improve performance to an ACQA measure. An example of a qualifying project is a practice’s effort to improve its breast cancer screening score through the implementation of a new patient outreach process. The infrastructure investment application can be used to request funding for expenditures related to projects that will improve performance to ACQA measures. An example of a qualifying infrastructure request is an application for funding to purchase a retinal camera for the purpose of improving performance to the diabetic eye exam measure.
