Overview

Accountable Health Partners recognizes that behavioral and mental health are vital components of the overall health and wellbeing of our patients and community practices. The AHP Behavioral Health Integration Team (BHIT) serves as a primary source of practice support, providing education and training, patient/family resources, and up to date, evidence-based tools designed to assist you in screening and addressing behavioral health needs across the lifespan.

Collaborative Care Model for Behavioral Health (CoCM)

The Collaborative Care Model (CoCM) centers the primary care provider’s role in caring for patients with behavioral health needs by placing mental health services, either in-person or virtually, in the primary care practice. AHP supports this model at various practices across our network, and is interested in supporting interested practices in exploring this model and establishing new programs. Please reach out to George Nasra at George_Nasra@urmc.rochester.edu if you’d like to learn more.
