AHP Welcomes New Social Workers to Care Management Team

As part of our ongoing commitment to offering providers care management resources that complement their efforts to deliver high quality patient care, AHP is pleased to announce the addition of three new social work members of our care management team:

Emma Bocchino, MSW
Emma Bocchino completed her Masters in Social Work through the Greater Rochester Collaborative Program and brings diverse experience in working with high risk and complex children and adolescents across a variety of settings. She has worked as a Medicaid Service Coordinator with developmentally disabled individuals with a specific focus on those with intellectual and/or physical disabilities who engaged in high risk behaviors. Emma has also worked with pregnant and parenting teens and youth in a transitional living program where she provides assistance with stable housing, referrals to outside clinical supports and skill building for parents transitioning out into the community in the city of Rochester. Emma has also worked in the Comprehensive Psychiatric Evaluation Program and participated in comprehensive interdisciplinary collaboration, clinical assessment and diagnostic formulation and treatment planning in the areas of safety and high risk behaviors. Emma is excited to continue her work with adolescents through AHP to grow and expand her clinical comprehension and application in the field.

Lynne Hefferon, LMSW
Lynne Hefferon, LMSW enthusiastically joins AHP with an extensive history and background in medical social work. She most recently developed social work care management for a primary care academic practice, initiating services in the clinic setting. Lynne joins us with hospital social work experience, as well as community based services. She returned to academia after years of work in the field, to obtain her graduate degree. Strengths and passions include family systems, and best practice research for effective clinical, and solution focused therapy and care management.

Nichole Spring, LMSW
Nikki Spring, LMSW, CLC joins AHP with over 13 years working in maternal child health, primarily in home based programming, in the Rochester community. She has a strong background in both program development and providing direct services, as well as over ten years’ experience supervising a unique team of community health workers and social workers in grant-funded initiatives. In 2018 she transitioned to working primarily in the outpatient health setting and has had the opportunity to work in ob-gyn and developmental pediatrics offices enhancing her skills in clinic-based interdisciplinary collaborative team work. Nikki is passionate about staff development and training and is excited to continue to grow her clinical skills in her new role with AHP.
