Depression Screening and Follow-Up Resources
May 31st, 2022 | News•Network NewsDepression Screening and Follow-Up are report-only measures for the 2022 quality program year as part of an ongoing collaboration between AHP and Excellus Health Plan. To assist our providers in their efforts to screen patients and provide timely follow-up for patients testing positive, the AHP Behavioral Health Integration Team (AHP-BHIT) has created the Depression Screening Toolkit. The toolkit has helpful information and resources, like PHQ-2 and PHQ-9 sample tools, action plans related to scores on PHQ-2 and PHQ-9 tools, patient educational resources, and provider tools for crisis intervention and medication management.
