Free PCMH Consulting Available
July 31st, 2019 | Network NewsPatient-Centered Medical Home recognition from NCQA delivers many benefits to primary care practices, and for a limited time free consulting is available to assist practices with achieving recognition.
Common Ground Health contracted with New York State to deliver PCMH technical assistance through January 31, 2019, but practices can expect Common Ground’s support through March 31, 2019 if they contact Common Ground no later than mid-August to begin their PCMH project. Also for a limited time, the initial recognition fee for the NYS PCMH program is being offered to primary care practices at no charge.
In addition to overall better quality of care for patients, practices that earn PCMH recognition are eligible for significant incentives for their care of managed Medicaid and Medicaid fee-for-service patients.
