New Clinical Resource for Hypertension Blood Pressure Control
October 29th, 2021 | Network NewsAHP’s Optimization Team has developed a new clinical resource for Hypertension Blood Pressure Control. In this toolkit, providers will find links to the most up-to-date quality measure definition, resources from the American Heart Association, our new care management clinical pathway, and the HTN Management Pharmacy Pearl.
The Optimization Team is partnering with our Care Management Organization to implement a hypertension care management pathway in pilot practices this fall. Additionally, we are working to address inequities in Hypertension Blood Pressure Control with targeted interventions focusing on our Medicaid-insured population in 2022.
