Learn More about AHP’s Pediatric Outreach Team
April 30th, 2024 | Network NewsUnder the Pediatric Social Work Care Management umbrella, the Pediatric Outreach Team, has collaborated with care managers and their practices since we first introduced them to the network last May. They bring diverse experience in community-based work with children and families, including areas such as family services, domestic violence, children’s health home care, developmental disabilities, and behavioral health care.
Brianna Searight, BSW, and Meghan Willis, B.A, have been fielding referrals to support children and families facing barriers to accessing basic resources such as food, clothing, housing, financial and social services, insurance, infant care items, dental care, developmental evaluation, behavioral health and substance abuse counseling, and transportation.
Referrals to the Pediatric Outreach Team flow through embedded care managers, so there are no changes to practice workflows. Care managers will continue to collaborate with Brianna and Meghan as necessary to address the family’s basic needs.
