Looking Ahead: New Tools and Health Catalyst Integration Strengthen AHP’s Data Coordination in 2025
May 30th, 2025 | NewsWe’re excited to share an update from AHP’s Data Coordination program as we head into a new year of collaboration and shared progress. In 2025, we’re introducing enhanced tools and beginning to use Health Catalyst—a powerful information technology platform that will help us better support your work by identifying population health opportunities tied to our value-based contracts.
This year, we’re focusing on two main priorities: supporting the closure of care gaps identified through our performance data and improving how we identify and respond to health-related social needs across our community. To do this, we’re strengthening our partnerships with AHP Care Management and Forward Leading IPA, our Social Care Network, to ensure a more connected, patient-centered approach.
While the heart of data coordination remains the same, we’re evolving how we do the work and moving beyond sole reliance on practice EMRs. Instead, we are using Health Catalyst to bring together both EMR and claims data. This more comprehensive view allows us to proactively identify care gaps and streamline patient outreach in ways that align more closely with our contracts and your workflows. We will be sharing additional information in the months ahead about how this process will be rolled out within your practice and for your patients
