As we close out the year, we will be focusing on Diabetic Eye Exams, Diabetic Nephropathy Screening, Hypertension Control and Falls Risk Screening within our Excellus Medicare contract. As has been discussed in the past, optimizing our metric performance not only demonstrates that our network provides excellent care to the patients we serve, but also positions us to maximize our shared savings in our value based contracts. We are currently hovering right at the threshold of maximal performance within our Excellus Medicare contract, and closing a small number of gaps will securely exceed the performance threshold. For this reason, we have begun to reach out to practices to ensure we close these gaps of patients who are already scheduled for visits before year-end. Our team will be reaching out to your offices and Care Managers to identify patients and finish strong in these metrics!
