The federal government has expanded telehealth access and extended several key Medicare flexibilities. Some provisions are now permanent, while others remain in place through December 31, 2027

  • Medicare patients can continue receiving non-behavioral/mental health telehealth services from home through the end of 2027.
  • Geographic restrictions are waived, allowing telehealth services regardless of patient location.
  • All eligible Medicare providers may continue delivering telehealth services through 2027.
  • Federally Qualified Health Centers (FQHCs) and Rural Health Clinics (RHCs) can serve as distant site providers for non-behavioral/mental telehealth services through 2027.
  • The requirement for an in-person visit within six months of an initial behavioral/mental telehealth service—and annually thereafter—is waived through 2027.
  • Audio-only telehealth remains permitted for non-behavioral/mental services through 2027. Additionally, audio-only communication may be used permanently when the provider can offer video but the patient cannot or declines it.

These extensions support continued access to telehealth services, particularly for rural and underserved populations, and provide practices with ongoing flexibility in care delivery.

How This May Impact Your Practice

  • Greater flexibility to provide care to patients regardless of their location, including those in urban areas or at home
  • More clinicians within your organization may be eligible to deliver telehealth services, supporting improved access and scheduling efficiency
  • Continued ability for Federally Qualified Health Centers and Rural Health Clinics to bill as distant-site providers, helping sustain telehealth revenue
  • Reduced administrative burden related to in-person requirements for tele-mental health visits
  • Ongoing opportunity to use audio-only visits for patients who lack video capability, improving access for underserved populations

Centers for Medicare and Medicaid Services (CMS) has created a Telehealth FAQ to answer any questions you may have about these new guidelines.
