Moving Forward: AHP’s Health Catalyst Transition
March 21st, 2024 | Network NewsAccountable Health Partners has begun the multiphasic transition to Health Catalyst as our new population health management vendor. The AHP team is working closely with our new partners at Health Catalyst to explore our new tool, find improved ways to share data with our network, and enhance how we interact with practices and providers to advance our shared goal of improving the quality of care for patients in our community.
While we expect most practices to be connected to Health Catalyst in 2024, the transformation project will continue into early 2025. Some practices have already received communication from AHP to kick off their transitions. AHP will continue to contact practices regarding their transition as the project progresses. We ask that you respond to requests related to your practice’s Health Catalyst transition as soon as possible.
