John R. Williford, CEO, Accountable Health Partners

One year ago, I stepped into the role of Chief Executive Officer of Accountable Health Partners with a clear vision: to build on AHP’s strong foundation and advance value-based care and population health across our region. Over the past twelve months, that vision has begun to take shape through the collective efforts of our providers, hospitals, practices, and AHP team members.

As I reflect on the past year, I am proud of what we’ve accomplished together.

Expanding Value-Based Care Opportunities

AHP successfully launched its first performance year in ACO REACH, one of Medicare’s most advanced total-cost-of-care models, in partnership with Pearl Health. The results speak to the strength of our network: AHP achieved top-tier quality performance in both the first and second quarters of the year — a direct reflection of the care our providers deliver every day. Building on that success, we have extended the partnership through 2028, positioning the network for continued growth and innovation.

The organization also secured important payer agreements, including the 2026–2028 Excellus Commercial contract renewal, Excellus Medicare amendments for 2026, and a new Aetna Medicare Advantage agreement, strengthening AHP’s value-based portfolio and creating new opportunities for participating providers.

Supporting Our Practices

Value-based care succeeds at the practice level, and AHP continues to invest in the resources that make that possible. Our care management and pharmacy teams work alongside practices every day to support patients with complex needs, close gaps in care, and improve medication management. Practices today have access to self-service quality dashboards, and we are enhancing that support with proactive quality monitoring and management reporting — launching in late 2026 into early 2027 — that will give practices earlier, more actionable insight into their performance, so opportunities are identified and addressed in real time.

Strengthening Our Voice in Healthcare Transformation

AHP expanded its engagement with state and federal policymakers this year, contributing formal comments on the New York State 1115 Medicaid Waiver, the NYS Value Based Payment Roadmap, and federal Medicare rulemaking. Through these efforts and ongoing collaboration with the New York State Department of Health, AHP has strengthened its role as a trusted voice in healthcare transformation and rural health innovation.

Investing in Rural Communities

AHP co-submitted eleven applications with the UofR Medicine to the NYS Rural Community Health Integration Program across five counties and three innovative care models. These projects represent a significant commitment to improving access, coordination, and quality of care in rural communities throughout our region. Recently, UofR Medicine was awarded $6.5 million in Rural Health Transformation funding, helping advance this important work.

Building the Infrastructure for Long-Term Success

AHP continued to invest in the people, technology, and analytics that support population health management. Enhancements to the Health Catalyst Enterprise Data Warehouse, Tableau reporting capabilities, and data infrastructure are providing stronger insights and helping translate clinical excellence into measurable outcomes for patients, providers, and payers.

At the same time, AHP expanded its team across analytics, clinical programs, and operations, ensuring the organization has the expertise and capacity needed to support a growing network and evolving healthcare landscape.

Looking Ahead

The progress achieved over the past year reflects the strength of the AHP network. Nearly 4,500 providers and 15 hospitals work together to improve the health of the communities we serve, and that collaboration remains our greatest asset.

As we look to the future, AHP remains focused on being the region’s indispensable partner for value-based care and population health. New partnerships, innovative programs, and expanded opportunities are on the horizon, and we are excited to continue advancing this work together.

Thank you for your partnership, commitment, and shared dedication to transforming healthcare across our region. More good work lies ahead, and I look forward to doing it with you.
