Pharmacy Team

Clinical Pharmacy Services

Accountable Health Partners has built a strong clinical pharmacy program to help patients get the most from their medications. Our clinical pharmacists work as part of the care team in primary care offices across the network. They partner with physicians, advanced practice providers, nurses, care managers, and office staff to make sure each patient’s medications are safe, effective, and easy to use.

Our Philosophy of Practice
“We, the primary care clinical pharmacy team, deliver patient-centered comprehensive medication management that ensures every medication is appropriate, effective, safe, and aligned with each patient’s goals and lived experiences. We work closely with the healthcare team to remove barriers, support informed decision-making, and improve access to high-quality care. Grounded in empathy, evidence-based practice, and shared accountability, we focus on optimizing outcomes, building trust through trauma-informed care, and strengthening the health of our community.”

Our main service, Comprehensive Medication Management (CMM), goes beyond a one-time visit. Clinical pharmacists review all medications to ensure each is necessary, effective, and safe, provide ongoing follow-up to help patients meet treatment goals, and address medication-related concerns. They care for patients with complex chronic conditions such as diabetes, high blood pressure, heart and lung disease, and depression; working closely with providers to adjust therapy and improve outcomes. The team also offers drug information support, educates care teams, and promotes safe transitions after hospital visits. As we grow, we are expanding to include centralized clinical pharmacy support to strengthen coordination across the AHP network, share best practices, and ensure consistent, high-quality clinical pharmacy services for all patients.

Benefits to Patients and Providers:

  • Improved Health Outcomes: More patients reach their blood pressure, blood sugar, and other health goals.
  • Safer Medication Use: Clinical pharmacists identify and resolve medication problems before they lead to harm.
  • Stronger Care Teams: Real-time collaboration improves decision-making and reduces provider workload.
  • Better Patient Experience: Patients feel supported, informed, and confident in their treatment plans.

AHP’s clinical pharmacy program plays an important role in delivering patient-centered, team-based care that improves health across our network.

Current Pharmacy Program Initiatives/ PCP Reward Plan components.

The PCP reward plan

The PCP reward plan is designed to align financial incentives with the delivery of enhanced patient care, encouraging providers to prioritize effective and efficient treatments. By focusing on high-quality care, the plan aims to improve patient outcomes, reduce hospital readmissions, and enhance overall patient satisfaction. Simultaneously, managing costs ensures the sustainability of healthcare systems by avoiding unnecessary procedures and promoting preventive care. Ultimately, high results lead to financial rewards, motivating PCPs to achieve these dual goals. Providers are incentivized to innovate and optimize care processes, knowing that their efforts can lead to tangible rewards. This collaborative goal not only benefits patients through improved care but also rewards providers for their contributions to a more efficient and effective healthcare system. Utilizing the Provider Engagement Report (PER) to reach maximum incentives

The Provider Engagement Report (PER) is a tool offered to providers that affords the opportunity to drill down to patient-level data related to important pharmacy initiatives. The Pharmacy Costs Dashboard within the PER gives providers a quick view of their initiative status; highlighting patient opportunities and medication-specific data.
