In this month’s population health update AHP maintains both its upward trend for preventive and adult disease management categories and top percentile performance in the pediatric and adult acute measures. With the implementation of the AHP Care Management Platform (covered in our April Network News) and along with it the new diabetes care management protocol, we will have powerful new tools to support further improvement in our adult disease management measures.
