Population Health Innovation Grants

As previously announced, AHP released a call for applications for our Population Health Innovation Awards program in the hopes of funding grassroots initiatives to improve care delivery and reduce costs of care for the population we serve. We are pleased to report that we received over 20 different submissions spanning the full scope of AHP’s membership. We have a committee reviewing these proposals and anticipate announcing our awardees soon. We will both announce the awardees and the title of their proposals in next month’s AHP⁵ and will keep you abreast of status updates as these programs roll-out in the year ahead.
