AHP’s negotiations with payers around 2024 quality metrics have concluded. The measures to which the network will perform will be distributed in the coming weeks along with plans for communicating performance during our transition to Health Catalyst as our new population health management vendor, and a review of all of the 2024 PCP Reward Plan components. As in prior years, providers can expect to see a measure grid focused on adult and pediatric population health with a limited number of new measures.
AHP has developed several resources to help providers meet our cost and quality goals, including:
- Well Child Roadmaps: An excellent resource for parents of young children that highlights the importance of well-child visits and comprehensive screening.
- Pharmacy Pearls: Easily digestible newsletters covering various pharmacy topics to help providers navigate shortages, prescription switches, and best practices for medications related to specific disease states.
- Provider Engagement Report: A dynamic, electronically delivered tool that allows providers to access a holistic view of their Reward Plan performance.
