Quality Metrics Icon

AHP remains dedicated to continuously improving patient care, and our quality measures are a direct reflection of this commitment. We are pleased to highlight the elements of the 2026 Quality Program included in the PCP Reward Plan. High performance in these areas will not only enhance the quality of care for your patients but also optimize the incentives you can earn under the plan.

A Return to Provider Performance

In 2026, the PCP Reward Plan will return to individual provider quality measure performance, leveraging our now fully integrated population health management platform, Health Catalyst.

Key Measure Updates for 2026

While the quality grid remains largely consistent with previous years, please note the following important changes for 2026:

  • Pediatric Measures: We have added Child & Adolescent Well Care Visits (ages 3–21) to the pediatric quality grid.
  • Breast Cancer Screening: The lower age limit for this measure has changed from 52 to 42 years old.

A full list of quality measures can be found on our website: ahpnetwork.com/quality-program/quality-measures/

Support and Resources

AHP continues to offer a variety of resources to help your practice meet cost and quality goals, including:

  • Well Child Roadmaps: A resource for parents highlighting the importance of screenings and well-child visits.
  • Pharmacy Pearls: Concise newsletters to help providers navigate medication best practices and shortages.

Tools for Tracking Success

To support this transition, we successfully rolled out an improved Quality Measures Dashboard via the Provider Engagement Report (PER), a dynamic electronic tool providing a holistic view of your Reward Plan performance. This tool allows you to monitor and track performance in real-time to reach 2026 goals.

To view your data:

  1. Navigate to AHPNetwork.com.
  2. Locate the Provider Engagement Report.
  3. Log in based on your specific practice type.

Need Access?

All PER users need special security permissions to access the PER dashboard and reports. To begin the process of obtaining user credentials or to request Well Child Roadmaps for your practice, or for assistance with other AHP resources, please reach out to your Provider Relations Representative, Molly Parmeter or Salena Sachman.
