Readmissions Pilot Project Launched at Highland Hospital
July 18th, 2019 | Clinical ConnectionsAHP, along with Highland Hospital, has launched a Readmissions Reduction Initiative Pilot aimed at reduced all-cause readmissions by 15%. The pilot’s model bridges the inpatient and outpatient environments through a series of six interventions designed to help keep patients healthy at home. The pilot launched on July 1, 2019, and we anticipate a six month phase during which we will continually assess the process and determine the future course of the project. This is an exciting demonstration of AHP’s population health efforts, and we are very glad to be partnering with Highland Hospital on such an important initiative.
