Reboot On Diabetic Nephropathy Measure Efforts

As a network, AHP has fallen in our performance on the Medical Attention for Nephropathy metric. However, we are extremely close to reaching the 90th percentile; there are less than 500 patients across all practices that need gap closures to get us there.

As a reminder the definition for this measure is as follows: The percentage of patients 18–75 years of age with diabetes (type 1 and type 2) with a nephropathy screening test or evidence of nephropathy, defined as evidence of treatment for nephropathy, evidence of a visit to a nephrologist, a micro albumin test, an ACEI, ARB, or a positive macro albumin test in the measurement year.

In the coming weeks we will be reaching out to engage practice in efforts to improve our performance on this measure.
