Network Rollout of the PCP Reward Plan and PER

The Provider Engagement Report (PER) is now available for implementation for all Arcadia-implemented primary care practices during the month of October.  The PER is a dynamic report developed by AHP to support provider efforts to achieve excellent quality outcomes within the financial performance thresholds of the network’s value-based contracts and the new PCP Reward plan. The new tool is delivered electronically and offers providers the ability to drill down to patient-level data related to important clinical and cost-saving initiatives.

Four large-group training Reward Plan/PER training sessions have been scheduled:

During each session, members of AHP’s PER team will review the new PCP Reward Plan, demonstrate the tool’s many helpful features, and answer questions that users may have about the tool.  Providers or other practice team members that would like access to the PER are welcome to attend a session. No RSVP is necessary.

Although not necessary for attending the training, some users may wish to access the PER prior to attending a training session.  Obtaining a URMC guest account is the first step toward accessing the PER, so your provider relations liaison will be sending to your practice a user spreadsheet to begin that process.  Once the spreadsheet has been returned, AHP’s PER guest account sponsor, Sharee Turpin will initiate the guest account request and serve as the point of contact for the user set-up steps that follow.  Detailed information about the guest account process and an FAQ will accompany the user spreadsheet.
