Starting Jan. 1, 2026, United Healthcare began requiring members enrolled in several Medicare Advantage HMO and HMO-POS plans to obtain a referral from their primary care provider (PCP) before accessing certain specialist services in the outpatient, office, or home settings. A grace period of January 1 through April 30 will expire and the requirement will be enforced beginning with dates of service on and after May 1, 2026.

Specialty care providers are recommending referrals be sent for all United members regardless of plan to prevent any potential delays in care. Specialist-to-specialist referrals also require a referral from the patient’s PCP and all referrals will be valid for six months. Patients may lose their appointments or be placed on waitlists for specialties with extended scheduling timelines if a referral cannot be obtained.

Additional information on the change can be found here.
