Enhancing Care Coordination Through Improved Documentation Workflows in Epic: We Want Your Input

As part of our ongoing commitment to improving care coordination and enhancing workflow efficiency, AHP’s Care Management teams will soon implement new documentation workflows in the Epic electronic medical record. These enhancements are designed to better support the delivery of integrated, patient-centered care and align with the standards of the Patient-Centered Medical Home (PCMH) model. By strengthening our documentation practices, we’re reinforcing our shared goal of providing comprehensive, coordinated care across the care team. While the new workflows will introduce some additional documentation requirements, we are approaching this transition with a strong emphasis on collaboration, transparency, and thoughtful implementation.

A Shift Toward Centralized Documentation

Historically, our dedicated care managers have managed the complex task of documenting across multiple systems, prioritizing a “practice EMR first” approach while also contributing to shared care delivery models, resulting in some redundant or inefficient processes that we will improve or eliminate with the transition to Epic. This longstanding structure has served us well, but has also created redundancies and inefficiencies that can impact both staff workload and patient outcomes.

Documentation in Epic will allow streamlined communication, reduce duplication, and ensure that key care information is easily accessible to all members of the care team, bettering staff workload and patient outcomes. AHP is committed to navigating this change thoughtfully and collaboratively to define new workflows and processes.
