Meet AHP Care Manager Tiffany King

The network’s care management program was developed to do just that, and since its inception earlier this year, AHP has funded the placement of many care managers into community-based practices requesting such support.
The story below is one in a series of articles that profiles members of AHP’s care management team and highlights their important work. This month, we profile Tiffany King, BSN, RN who joined AHP in January 2016. She is embedded in the office of Alexander Medical Group in Rochester.
Tiffany’s career began in Public Relations and marketing after graduating from Hofstra University in 2004. Working for three years in the advertising department at The Stop and Shop headquarters in Quincy, MA, Tiffany felt that she wanted to find a career in which she could make a positive impact on the lives of others. It is at that time that she decided to become a nurse. She graduated from Regis College in Weston, MA and quickly moved to Fairport NY to start her career as a nurse on 7-1200, an ENT and thoracic surgical unit at Strong Memorial Hospital. From there, Tiffany moved into a clinical care manager roll, working for the chief of surgery. It was during that experience, that she realized her passion of helping people navigate their way through the complex health care system.
While at Strong, Tiffany transitioned into an inpatient care coordinator role on a very busy short-stay unit which managed the care of a large, underserved population. She learned about the challenges of managing chronic illness and witnessed the barriers that patients often meet. Tiffany realized that care management was an area within the nursing profession that she truly enjoyed and she wanted to build upon her knowledge base by continuing her education at The University of Rochester, School of Nursing. Currently, she is working towards her Master’s degree in Leadership and Healthcare Management, graduating in May 2018. Tiffany writes,
“My time at AHP has been such an enriching experience. The group of people that I work with and the support that we receive has been invaluable. Managing care in an outpatient setting has brought my career full circle and I am grateful for the opportunity to help patient’s work towards their health care goals. I look forward to continuing on this path, learning and growing professionally.”
