FLPPS Innovation Fund Call for Proposals
August 30th, 2017 | ArchiveFLPPS-contracted practices have a unique opportunity to submit applications for funding for quality improvement projects that are not under existing project requirements but that still support a DSRIP clinical outcome metric. A $2.5 million Innovation Fund was established to support initiatives that will contribute to the achievement of the performance targets of select metrics, including antidepressant medication management, low birth weight, adult access to primary care and childhood immunization status, among others. Project proposals must include the following:
• Executive summary
• Targeted clinical outcome(s)
• Cost of project and intended financial and clinical impact
• Timeline to implementation
• Measurable reporting
• Plan for project’s sustainability
