Learning Collaboratives: Stay Informed and Share Best Practices
August 27th, 2018 | ArchiveLooking for a small group to meet with and discuss the real life issues you deal with every day? Hoping to connect with other healthcare workers that understand the challenges and rewards of our chosen career? Look no further! AHP has several Learning Collaboratives that meet monthly to discuss these issues and brainstorm ways to overcome obstacles.
Each meeting consists of three main agenda items: updates on state and federal programs (such as NYS PCMH and MIPS) and AHP news; an educational piece focusing on change management or a specific measure, for example; and a time to bring a challenge that you are facing to a group of sympathetic listeners for brainstorming and useful ideas. We ask a lot of questions in an effort to make positive changes, and often use that as a strategy to find the most helpful solution to issues that arise in our community practice sites.
Sally Smith, Practice Manager for Thompson Health community practices, says, “I find it helpful because I find out through our discussions that our practices are not the only practices going through some of the changes that healthcare is demanding. When our practices find an issue frustrating I find out from attending that other practices are struggling too.”
Communication styles have been a popular topic in 2018, and have been helpful to Sue Robshaw, a care manager in the UR Medicine Primary Care Network. She says, “The Learning Collaborative and Linda herself have assisted me in being able to take multiple (researched-based) approaches to communicating with different clients. Guiding clients through health decisions and lifestyle changes without becoming overly directive is an art in communication. The Learning Collaborative gives educators the tools to best navigate these discussions to enhance communication with clients.”
Our agendas for the remainder of the year for adult and family care practices are as follows:
September – motivation and engagement strategies for your staff
October – practice transformation tools and strategies
November and December – we will not meet due to the Thanksgiving and Christmas holidays
