Heart Failure Pilot Summary

Beginning in April, five practices from across the AHP network began participating in a pilot of our Heart Failure Utilization pilot. Genesee Valley Family Medicine, Internal Medicine Associates of Brighton, Olsan Medical Group, Southview Internal Medicine and Victor Health Associates all participated, and their amazing care management team led the charge. We are pleased to report that thanks to their efforts, they were able to demonstrate a 38.5% reduction in heart failure admissions and a 20% reduction in ED utilization for heart failure during the last several months! This is a tremendous accomplishment, and could not have occurred without their patience and tireless efforts. We greatly appreciate their participation and collaboration, and are beginning efforts to try to disseminate these best practices across the network to improve care for this fragile population of patients.

These early results received statewide praise when they were presented at the recent Excellus Value Based Care Alliance meeting in Syracuse this week so kudos to these groups for being leaders in effective care management and care delivery in our region!
