Diabetic Nephropathy Metric Performance Improvement Plan
September 20th, 2019 | ArchiveAs we have crossed the halfway point of our contract year, we have assessed our performance across all contracts and lines of business to ensure that we remain on track to maximize our quality performance and optimize our shared savings. While we continue to do well, there remains room for improvement, particularly in the area of diabetic nephropathy screening where our performance has wavered compared to last year.
Based upon our practice resource inventory that almost all of you completed for us earlier this year, it appears we have the resources in most of the practices to easily close these gaps if and when the patient attends his/her normally scheduled visits. Therefore, to help facilitate accelerated gap closure with this metric, and improve our performance across ALL lines of business, we have generated chase lists of patients with this specific gap and who have appointments scheduled in our primary care practices between now and year’s end. We ask that each site use these chase lists in your pre-visit planning to ensure gap closure. Your practice managers and care managers have either already received or will be receiving these lists shortly to aid in this endeavor. We appreciate your continued partnership as we make a push to finish our year with a strong performance!
