The Patient Centered Medical Home (PCMH) is a model of care delivery that emphasizes care coordination and communication to improve patients’ experience of care and medical outcomes, all of which complement the AHP mission to improve quality and lower certain healthcare costs. The network has supported physician efforts to obtain PCMH recognition from NCQA through its consulting project that provides participating practices with access to an RMS Healthcare consultant. These skilled and experienced professionals assist practices with the transformation process by developing work plans, facilitating the process of meeting plan objectives, and organizing the submission of documentation and applications.
