Meet AHP Care Manager Danielle Calder

Danielle Calder, MS, RN-BC began as the AHP care manager for Victor Health Associates in March 2015. Danielle earned her baccalaureate degree in nursing and then her Master’s degree in health care organizational management and leadership at the University of Rochester School Of Nursing. Her technological and nursing leadership expertise position Danielle well to collaborate effectively with her Victor Health colleagues in skillfully managing the health of the population served by the practice.
Danielle reflects on her nursing and complex care management experience as follows:
I began my career with an Associate’s degree in nursing from Monroe Community College. Intent on earning an advanced degree, I continued my education while working as a registered nurse at Strong Memorial Hospital and in 2012 was awarded board certification in medical-surgical nursing by the American Nurses Credentialing Center. I completed my baccalaureate degree in 2013 and this past May, I graduated with my Master’s degree in health care organizational management and leadership from the University of Rochester School of Nursing.

I have always been an early adopter of change and have participated in several initiatives focused on strengthening health systems and utilizing technology to improve care delivery processes. As AHP continues to provide innovative tools for physicians to support their practices, I am eager to assist Victor Health Associates in the utilization of Arcadia and other technology improvements that can significantly help us manage the health of our practice population. I am also currently working with an interprofessional team from the University of Rochester Medical Center and AHP to assess and develop recommendations regarding how technology can be used more effectively to support seamless care coordination across the health care system.

Prior to joining AHP, I was an inpatient care coordinator, which I believe enabled me to discover my passion in nursing. The goal of this role was to comprehensively plan for patients who were being discharged from the hospital setting and to ensure that they were equipped to be successful at home. Assisting patients in navigating the health care system and making certain that they had the best transition to home was important to me. I thoroughly enjoyed equipping patients to successfully self-manage and to support them post-hospitalization through comprehensive care coordination including linkages with essential community resources that assisted patients in enjoying the highest quality of life possible where they wanted to be—at home.

Advocating for patients, as well as encouraging them to make informed decisions that help them to take control of their health are some of the reasons underlying my love for this role. I am excited to use the skills that I have gained and build upon them as a care manager at Victor Health Associates to positively influence patients’ experiences and connection to their health care. For me, care management goes beyond coordinating resources, assisting with transitions, population health and disease management. Care management provides me with myriad opportunities to develop meaningful relationships with patients, to let them know that someone else cares about them, and to empower them to be successful in reaching their health goals.
