AHP’s Care Management Teams Transitioning to Epic’s Compass Rose Platform

Over the next several months, AHP’s Social Work and RN Care Management teams will transition to Epic’s Compass Rose (CoRo) as their primary platform for documenting patient encounters, care plans, and collaboration with specialty providers. Compass Rose is Epic’s Coordinated Care Management toolset that will support standardizing how our care managers and social workers identify and enroll patients into programs, coordinate care between providers, and perform continued outreach and follow-up.  Compass Rose will enable us to monitor program milestones and patient outcomes impacted by care management interventions and inform future population health priorities. This transition is closely aligned and supports AHP’s transition to Health Catalyst as our new population health management platform.

 

This transition to Compass Rose began on May 19, 2024, and will continue with a phased approach through 2025. In Summer 2024, a workgroup will be convened to capture preferred workflows and feedback from practice care teams. Practice managers and providers who will be directly impacted by this change were notified via email communication.
