As we reflect on the past year in this final Network Newsletter of 2025, we are filled with gratitude and pride. Together, we’ve navigated another year of growth, collaboration, and meaningful progress across AHP. The milestones we’ve reached are a direct result of the commitment and dedication of our network, and they set the stage for an even more transformative year ahead.

Looking toward 2026, we are excited to share several major initiatives that will strengthen AHP’s infrastructure, elevate the support we provide to our practices, and enhance the care we deliver to our communities. Among the work already underway are:

The Launch of Our New Website
A redesigned digital experience aimed at improving navigation, transparency, and access to the tools and information you rely on.

Health Catalyst Implementation
The rollout of Health Catalyst will significantly elevate our data and analytics capabilities, giving your patients the benefit of more proactive, coordinated, and informed care. By strengthening our ability to identify trends, risk factors, and gaps in care, we can ensure patients receive timely interventions and more seamless support across the network.

For you as providers, Health Catalyst is designed to reduce administrative burden and make performance insights more accessible and meaningful. It will streamline reporting, decrease manual data collection, and enhance decision support—allowing you to focus more time and attention on your clinical work and less on navigating fragmented information systems.

Social Care Network (SCN)
The SCN will help us collectively address the social determinants of health that often stand in the way of positive outcomes. Patients struggling with food insecurity, housing instability, transportation challenges, or other social needs will have better access to coordinated community resources, enabling more comprehensive and equitable care.

Our goal is to give you a reliable, structured way to respond when social needs surface in the clinical setting. Instead of relying on informal or ad hoc solutions, you will have a consistent system to screen patients, track progress, and ensure follow-through—ultimately supporting better disease management and improving patient engagement.

ACO REACH
ACO REACH strengthens our commitment to value-based care and places a strong emphasis on health equity. Your patients, particularly those who are high-risk or underserved, will benefit from improved care coordination, enhanced preventive services, and reduced fragmentation across settings.

This program will provide additional care-management support, better insight into patient needs, and increased flexibility in how care is delivered. By aligning incentives around outcomes rather than volume, ACO REACH helps ensure that the resources you need to provide comprehensive, patient-centered care are more readily available.
