Care Management Program Delivering Value

AHP’s care management program is well underway, with a number of community-based practices benefiting from either embedded care management resources or funding for existing care managers.

The care management program supports physician efforts to better manage care transitions and complex patients with chronic disease and in turn, improve performance on the clinical measures associated with AHP’s gain-sharing contracts. Our growing team of six care managers and one LCSW currently supports eight adult primary care and six pediatric practices, while the network provides support for care managers in six additional practices that were part of the Finger Lakes Health Systems Agency’s CMMI grant. Rounding out our care management “community” are the 39 care managers working in AHP hospital-employed PCP practices. Expanding the program to additional community-based PCP practices is a key deliverable for Daryl Sharp, AHP’s Director of Care Management as she continues to recruit baccalaureate RNs to fill open positions, particularly in Allegany, Livingston, Steuben, and Wyoming Counties.
