UR Medicine Opens Complex Care Center

Patients who have been coping with serious health problems since childhood now have a health resource designed just for them at UR Medicine’s Complex Care Center, which opened on March 21st at 905 Culver Road, Rochester. The Center will be Rochester’s first primary care practice dedicated to caring for adults with chronic childhood-onset conditions such as cystic fibrosis, sickle cell anemia, childhood cancers, autism, and intellectual and developmental disabilities.

The Center will offer specialized outpatient services to better meet the needs of patients who often have multiple, severe conditions that require close management, and who face disparities in accessing health care. These patients often have difficulty finding an adult primary care provider, and lack of reliable access to outpatient care often results in preventable Emergency Department visits and hospital stays.

As AHP works on initiatives that support better transitions for patients moving from pediatric to adult primary care, the Complex Care Center will be an important resource for those most complex members of this population. To learn more about the Complex Care Center, call 585-276-7900 or click here.
