December 21st, 2015 | ArchiveAs this busy year draws to a close at AHP, we must pause to express our gratitude to our provider partners for their many contributions to our success as a network. When we reflect on where AHP started in 2013 and where we are today, our growth and success is due in such large part to your efforts.
Many of you and your staff have given of your time to achieve implementation of the Arcadia dashboard in your practice; others have welcomed AHP care managers on to their care teams or have agreed to undergo the intense process of practice transformation that leads to Patient Centered Medical Home recognition. Still more of you have attended 6:30 am (or pm!) meetings to give us the gift of your experience and insights.
Please accept our thanks for all of your efforts, and our wishes for a bright holiday season spent with family and friends. We look forward to working with you in 2016!
