Health Catalyst Update: Continuing the Implementation of AHP’s New Population Health Management System
August 28th, 2024 | Quality•NewsWe are thrilled to communicate the continued work surrounding implementing our new population health management system, Health Catalyst. Here are some key milestones and achievements that have been accomplished thus far:
Transition to Health Catalyst
- The AHP Arcadia contract concluded on May 31, 2024, ending our network’s use of the platform. Users who had access to Arcadia through AHP, can no longer access the platform
- We continue to work with a few practices to switch their EMR feeds from AHP’s Arcadia environment to the Excellus Arcadia environment. This connection is required to accurately report the quality measure performance on our Excellus contracts.
- We are at various stages of implementation with each group beginning with the connection of the EMR to Health Catalyst.
Upcoming Milestones
- Quality measure performance will soon be turned on in the existing Provider Engagement Report (PER). As each practice comes online with Health Catalyst, the PER will connect to a new Quality Measure Dashboard for performance monitoring and patient lists.
While much has been accomplished, the rolling implementation will continue into 2025. We look forward to continuing our journey toward enhanced population health management with Health Catalyst, ensuring better care delivery and outcomes for our network.
