Whether it’s to help patients find the correct phone number for your office on AHP’s online directory or to ensure that UR health plan patients’ claims process at the right benefit level, AHP needs your help to keep your information up to date.
Please use the form found here to inform us of:
• A provider (physician, CNM, NP, PA) joining or leaving your practice
• Changes to your TaxID
• Changes to your address, phone and fax numbers
