The Countdown Continues: Provider Engagement Report
August 31st, 2021 | ArchiveAs reported in our last edition of Network News, the AHP team is developing a new tool to support provider efforts to achieve excellent quality outcomes within the financial performance thresholds of the network’s value-based contracts. Called the Provider Engagement Report, this dynamic report will be delivered electronically and offer providers the ability to drill down to patient-level data related to important clinical and pharmacy initiatives.
In the coming months, contract-participating private primary care practices can expect to be invited to information sessions about this exciting new tool. These specialty-specific Zoom meetings will provide an introduction to the new tool and its data sources, review the information AHP will need from each practice in order to grant access to the PER, outline the timeframes for training and implementation, and discuss the expectations for using the report and acting on the data it contains. Staff will also be reaching out to participating hospital systems to collaborate on the best strategies for implementing the PER for their employed PCPs.
