Call for Proposals: Pop Health Investment
September 7th, 2018 | ArchiveDue to AHP’s success in our most recent year’s value based contracts, we have the good fortune to have achieved significant shared savings. While the vast majority has been distributed back to our members as gain share, we felt it would be prudent to utilize a portion of this money to fund initiatives aimed at improving one or more components of the “quadruple aim” in value-based care delivery (improved health of the patient, improved cost savings, improved patient satisfaction, and/or improved provider satisfaction). While we have been successful in launching multiple health improvement initiatives this year, we also realize that some of the best ideas for care and value transformation will come from the teams delivering care on a daily basis. Thus, we are announcing the launch of the AHP Population Health Investment Initiative where we are asking providers, care managers, allied health providers or any team working within the AHP network to submit brief proposals describing the initiative you are proposing, how this will improve value based care delivery, and a brief budget for the amount being requested and how that money will be used.
It is important to note that these funds are not intended to sponsor research. Likewise, while initiatives may include the hiring of additional staff in their budgetary request, those proposals without demonstrated sustainability of funding new hires will be adversely scored. Each proposal should be able to utilize the proposed funds and demonstrate intended performance within the 2019 calendar year. We anticipate the average award will be in the $25,000-$50,000 range, however we are willing to consider all proposals based on merit and potential for population health improvement, impact and sustainability. All initial proposals are due back by Monday, September 23, 2018 for review and we anticipate all awards will be distributed by December 31, 2018. Please note that applications can only be accepted from practices that participate in or are eligible to participate in all AHP payer contracts.
